Home health is skilled nursing and rehabilitative therapy provided in your home. Care is focused on helping you manage a chronic condition or recover from an acute illness, surgery, accident, or a change in medical condition.

Home health is covered 100% by Medicare, and we accept most forms of insurance. Part of the admission process includes verification of insurance. We can help answer your financial and insurance questions.

You or a family member is eligible for medical home health care services if you are:

  • Receiving treatment for an illness
  • Were discharged from the hospital recently and need services for continuity of care
    are homebound
  • Received recommendation from your physician for home health care services

The applications received at Therapeutic Home Healthcare go through our administrative staff. Those who are qualified will be shortlisted and will undergo a series of interviews, reference checks, background screening and skills assessment.
Once hired, the caregivers will be scheduled for orientation and training prior to being assigned to your home.

The frequency and type of home health visits are based on your personal plan of care. Your doctor may change your plan of care as necessary, increasing or decreasing the number of visits or services provided, to provide you with the best home health care for your needs.

We thoroughly screen and train our home health team members. Patient care staff have professional licenses and certifications that are applicable to their role. We also perform background checks and require several personal and professional references. Once hired, Links of Compassion employees continue honing their skills through an extensive orientation process and ongoing training programs

No. Hospitalization is not a requirement to receive home health care. Many patients are referred by their doctor.

All employees are updated weekly regarding their clients’ cases. Our nurses supervise Personal Care team members onsite, carefully monitoring the condition of the patient and reporting any changes in his/her physical condition to the family and/or physician.
